Description
Invalid Fee Schedule Code

Explanation

Updated October 26, 2026

The fee code is missing or in the wrong format. It must be a letter, three digits and a suffix A, B or C.

What it means

V40 ("Invalid Fee Schedule Code") means the service code is missing or isn't in the format ANNNA: a letter, then three digits from 001 to 999, then a suffix letter from A to C.

This is a format check: the claim was rejected before OHIP assessed the service itself, so it usually pays once the field is corrected.

The suffix is A when the physician did the service, B for a surgical assistant or technical component, and C for an anaesthetist or professional component (GP10).

Common causes

  • The fee code was left blank
  • The suffix letter was missing
  • A typing error, such as the letter O for zero

How to fix it

  1. Check the fee code on the claim.
  2. Correct it, including the suffix letter.
  3. Resubmit the corrected claim. Resubmit within three months of the service date if you can. If that window has passed but the claim was first submitted on time, use the Ministry's stale-dated resubmission process: one stale-dated claim file through MCEDT, then an email to [email protected] with the Error Reports showing the original submission date.

Don't resubmit if

  • You haven't changed anything. Sending the same claim again will reject the same way.

How APEX handles it
