Error code
V34 — Fee code not valid for this provider type
Wrong code for provider type
The fee code doesn't match the type of provider on the claim; physicians, nurse practitioners and labs each have their own codes.
What to do: Use a code your provider type can bill, or send the claim under the provider who did the service.
- Code
- V34
- Description
- Fee code not valid for this provider type
Explanation
Updated October 26, 2026
The fee code can't be billed by this type of provider. Use a code your provider type can bill, or have the right provider bill it.
What it means
V34 ("Invalid Service Code") means the fee code doesn't match the health care provider type on the claim.
For example, nurse practitioners, labs and physicians each have their own sets of codes.
Common causes
- A fee code from another provider type's schedule
- The claim was sent under the wrong provider number
- A typing error in the fee code
How to fix it
- Check the fee code and the provider number on the claim.
- Use the code your provider type is allowed to bill, or send the claim under the provider who did the service.
- Resubmit the corrected claim. Resubmit within three months of the service date if you can. If that window has passed but the claim was first submitted on time, use the Ministry's stale-dated resubmission process: one stale-dated claim file through MCEDT, then an email to [email protected] with the Error Reports showing the original submission date.
Don't resubmit if
- You haven't changed anything. Sending the same claim again will reject the same way.
How APEX handles it
APEX reads your error reports and remittance advice from MCEDT and lists claims rejected with this code in its Correction Centre for you to correct and resubmit. See how APEX works · Pricing
