Error code
V8 — Paid at a lower fee per OHIP policy
Paid at a lower fee
The service was paid, but at a lower fee than billed, because an OHIP payment policy called for the lower amount.
What to do: Compare the fee code, amount billed and amount paid against the Schedule of Benefits rules before deciding to ask for a review.
- Code
- V8
- Description
- Paid at a lower fee per OHIP policy
Explanation
Updated October 26, 2026
OHIP paid this service at a lower fee under one of its payment policies. Find which rule applied before deciding on an RAI.
What it means
V8 means the service was paid, but at a lower fee than billed, because an OHIP payment policy calls for the lower amount. The Ministry's full wording is that the service was paid at a lower fee as per stated OHIP policy; it doesn't name the policy.
Because V8 covers many rules, the Remittance Advice (RA) alone won't tell you which one applied. Look at the fee code, the other services on the claim, and the Schedule of Benefits payment rules for that code.
V8 is a reduction on a paid claim, not a rejection to the error report.
Common causes
- A Schedule of Benefits payment rule that pays the service at a reduced fee in the circumstances billed
- A rule that lowers the fee when the service is billed with another service
- A limit on the number of services reached for the code
How to fix it
- Note the fee code, the amount billed and the amount paid.
- Check the Schedule of Benefits payment rules and recent INFOBulletins for that code.
- If you find the rule and it applies, there is nothing to fix.
- If you can't tell why it was reduced, or believe the full fee applies, send a Remittance Advice Inquiry (RAI) through eSubmit, or on form 0918-84, within 7 months of the RA date. The Ministry's reply says whether it was paid correctly or needs an adjustment.
Don't resubmit if
- The claim was paid and is on the RA. Disputes go through an RAI.
- You want the full fee for the same service. A new claim would be a duplicate.
How APEX handles it
APEX has no correction rule for V8 and doesn't send inquiries for it. The line stays in your claims with the billed and paid amounts, so you can decide whether an RAI is worth sending. See how APEX works · Pricing
