Error code
VJ5 — Invalid Service Date
Invalid service date
The service date is missing, not 8 digits, impossible (such as 31 in a 30-day month) or later than the Ministry's run date.
What to do: Check the chart for the real service date, enter it as a valid 8-digit date, and resubmit.
- Code
- VJ5
- Description
- Invalid Service Date
Explanation
Updated October 26, 2026
VJ5 means the service date is missing, badly formatted, impossible, or later than the Ministry's run date. Enter a valid date and resubmit.
What it means
VJ5 means OHIP could not accept the date of service. The Ministry lists four reasons: the date is missing or is not 8 digits; the month is not 01 to 12; the day does not exist in that month; or the date is later than the Ministry's system run date (a future date).
Common causes
- The date was left blank
- A day that doesn't exist, such as 31 in a 30-day month or 29 February in a non-leap year
- Day and month swapped
- A future date
How to fix it
- Check the chart for the real date of service.
- Correct the date on the claim (8 digits, a real calendar date, not in the future).
- Resubmit the corrected claim. Resubmit within three months of the service date if you can. If that window has passed but the claim was first submitted on time, use the Ministry's stale-dated resubmission process: one stale-dated claim file through MCEDT, then an email to [email protected] with the Error Reports showing the original submission date.
Don't resubmit if
- The service has not happened yet.
