Explanation
Updated October 26, 2026
OHIP paid this eye exam as a major oculo-visual examination for a senior with a medical condition. Informational; usually no action.
What it means
Code VN means the eye exam was allowed as a major oculo-visual examination for a senior with a medical condition.
The Ministry's related code V5 says that for patients over 65 with a medical condition one major oculo-visual examination is allowed per 12 months, and for those without a medical condition one per 18 months. For physicians, a periodic OVA for patients 65 and over (A112) is limited to one per 12 months, counting exams by any physician or optometrist (GP28).
How to fix it
- Check that the amount paid matches what you expected.
- If it does, there is nothing to do.
- If you believe the payment is wrong, send a Remittance Advice Inquiry (RAI) through eSubmit, or on form 0918-84, within 7 months of the RA date, with supporting details.
Don't resubmit if
- You disagree with how a claim already on the RA was paid. Use an RAI, not a new claim.
