Explanation

Updated October 26, 2026

Enhanced 18-month well-baby visit: well-baby care plus a developmental screen and review of a parent-completed tool, age 17 to 24 months.

What it covers

A002 is billed when the physician does all of the following for a child aged 17 to 24 months: well-baby care (a complete examination with weight and measurements, and instructions to the parents); an 18-month age-appropriate developmental screen; and a review with the parent or caregiver of a brief standardized tool they completed that helps identify children at risk of a developmental disorder (GP34, A4).

The Schedule gives the Rourke Baby Record as an example of the screen and the Nipissing District Developmental Screen as an example of the parent tool (GP34).

Billing rules

  • Only for children aged 17 to 24 months (GP34).
  • Payable only when the chart records the developmental screen and any concerns found in the review of the parent-completed tool, in addition to the usual well-baby care record (GP34).
  • Detention (K001) can only start after 60 minutes (GP30).
  • A002 is not on the Appendix J list, so it can't be billed by video or telephone (A68, AJ1, AJ2).
  • Other well-baby visits in the first two years are billed as A007 (A4, GP34). Paediatricians bill A268 (A170).

Common rejections

  • A2A: The child wasn't 17 to 24 months old on the service date. Bill A007 well-baby care for other ages, and check the date of birth on the claim.
  • VJ7: OHIP received the claim more than three months after the service date (GP9). If it was first sent on time, use the stale-dated process.
  • EH2: The health card version code doesn't match the Ministry's record. Get the current version code and resubmit.
  • R02: OHIP's description is 'invalid HSN': the health number on the claim isn't valid. Check it against the child's card.
