Error code

A2A — Patient is outside the age limit for this fee code

Wrong age for code

The patient is too young or too old, on OHIP's records, for the fee code billed.

What to do: Work out the patient's age on the service date and bill the code for that age group.

Code
A2A
Description
Patient is outside the age limit for this fee code

Explanation

Updated October 26, 2026

A2A means the patient is too young or too old for the fee code billed. Bill the age-appropriate code and resubmit.

What it means

Some OHIP fee codes are tied to an age group. The Ministry's wording is that the patient is underage or overage for this service code. OHIP works out the patient's age from its records and rejects the claim if the code doesn't fit.

The Schedule of Benefits defines the age groups: newborn (up to 28 days), infant (29 days to under 2 years), child (2 to 15), adolescent (16 or 17) and adult (18 and older). Codes billed often by family physicians come in age-specific versions. Venipuncture is G480 (infant), G482 (child) or G489 (adolescent or adult). Periodic health visits are K017 (child), K130 (adolescent), K131 (adult 18 to 64) and K132 (adult 65 and older).

Common causes

  • Adult or child version of a code picked by mistake, often a code left in a template or favourites list
  • Patient recently moved into a new age group, for example turned 2, 16, 18 or 65
  • Date of birth or health number on the claim belongs to a different patient

How to fix it

  1. Check the patient's date of birth and work out their age on the service date.
  2. Look up the fee code's age group in the Schedule of Benefits.
  3. Replace it with the code for the patient's age group, and check whether the diagnostic code also needs to change.
  4. If the date of birth or health number on the claim was wrong, correct it.
  5. Resubmit the corrected claim. Resubmit within three months of the service date if you can. If that window has passed but the claim was first submitted on time, use the Ministry's stale-dated resubmission process: one stale-dated claim file through MCEDT, then an email to [email protected] with the Error Reports showing the original submission date.

Don't resubmit if

  • You haven't changed the fee code or the patient's details. The same claim will reject again.
  • The service has no version for the patient's age group. Check the Schedule of Benefits before billing something else.

How APEX handles it

APEX treats A2A as an automatic fix for venipuncture and wellness or assessment codes. It uses the patient's date of birth to swap in the age-appropriate code, then shows the claim for review, or sends it with auto-fix if you've turned that on. See how APEX works · Pricing
