Explanation
Updated October 26, 2026
Paediatrics (26) extended special consultation: a full paediatric consultation with at least 90 minutes of direct contact with the patient.
What it covers
A662 is a consultation in which the paediatrician provides all the elements of a paediatric consultation (A265) and spends at least 90 minutes in direct contact with the patient, not counting time spent on other separately billable services (A168).
The 90 minutes doesn't include non-patient-facing time such as reviewing charts or imaging, or documentation (A169).
Billing rules
- Start and stop times must be in the chart, or the service is not payable (A168).
- All consultation requirements apply: a written, signed referral naming the referrer with their billing number, kept in the chart, and a written report back. Otherwise it is paid as a lesser assessment (GP16).
- The consultation limits (one per two consecutive 12-month periods for the same diagnosis) apply to time-based consultations like A662 (GP17, GP18).
- Detention (K001) can only start after 180 minutes (GP31).
- Patients under 16: specialist consultation age premium of 10% to 30% (GP64).
- Video only (with K300A), never telephone (AJ2).
- For patients 18 or older, paediatricians may bill only if they saw the patient in the 12 months before the 18th birthday, ongoing chronic care is needed and the patient is under 22, or with the Ministry's written approval (A168).
Common rejections
- AC4: The referring provider number was missing, mistyped, your own number, or from a provider type that can't refer for this code. Enter the referrer's six-digit number from the request.
- VJ7: OHIP received the claim more than three months after the service date (GP9). If it was first sent on time, use the stale-dated process.
- A4D: The code is in the Paediatrics (26) listing, and specialists bill from their own specialty's listing (GP9). The specialty on the claim can't bill it.
- EH2: The health card version code doesn't match the Ministry's record. Get the current version code and resubmit.
- ERF: OHIP's description is 'referring physician not currently eligible to refer'. Check the referring provider number on the claim matches the provider who signed the written request.
