Explanation

Updated October 26, 2026

Psychiatric consultation for an adult with a complex neurodevelopmental condition, with at least 90 minutes with the patient and caregiver.

What it covers

A neurodevelopmental consultation includes all the elements of a consultation (A195) for an adult with complex neurodevelopmental conditions (for example autism or global developmental disorders), with at least 90 minutes in direct contact with the patient and caregiver, not counting other separately billable services (A183).

Consultations for children or adolescents, or for less complex conditions such as attention deficit disorder, are paid at a lesser fee (A183).

Billing rules

  • Record start and stop times in the chart (A183).
  • Maximum one per patient per physician every 5 years (A183).
  • Time excludes other separately payable services and non-patient-facing work such as chart review or documentation (A183).
  • Needs a written, signed request from a physician or nurse practitioner, kept in the chart, and a written report back, or it is paid as a lesser assessment (GP16).
  • Add K630 for each extra ½-hour unit once you pass 120 minutes with the patient (136 minutes for the first unit to be paid) (A185).
  • Detention (K001) is not payable for the same patient on the same day (GP31).
  • By video only (not phone) with an existing patient relationship; add K300A (Appendix J, A70).

Often billed with

  • K630 psychiatric consultation extension (A185).
  • K188 (+15%) within 6 months after a suicide attempt (A188).
  • K189 for urgent community follow-up within 4 weeks of a psychiatric discharge (A188).

Common rejections

  • ARF: OHIP describes this as 'Missing Physician Referring Number'. A consultation is paid only on a written referral from a physician or nurse practitioner (GP16); enter the referrer's billing number.
  • AT4: OHIP describes this as 'Modality Not Allowed': this code is on the video-only list, so it can't be billed with K301A (telephone) (Appendix J).
