Explanation

Psychiatric consultation for a patient 75 or older, with at least 75 minutes of direct contact, booked 24 hours ahead.
What it covers
A geriatric psychiatric consultation is payable to a psychiatrist for a patient aged 75 or older. It includes all the elements of a consultation (A195) and at least 75 minutes in direct contact with the patient, not counting time with caregivers or other separately billable services (A183).
It must be scheduled at least 24 hours before the visit (A183).
Billing rules
- Record start and stop times in the chart (A183).
- Maximum one per patient per physician every 5 years (A183).
- If the conditions aren't met, or the consultation is delegated to an intern, resident or fellow in a clinic teaching unit, it is paid as a lesser consultation or visit (A183).
- Time excludes other separately payable services and non-patient-facing work such as chart review or documentation (A183).
- Needs a written, signed request from a physician or nurse practitioner, kept in the chart, and a written report back, or it is paid as a lesser assessment (GP16).
- Add K630 for each extra ½-hour unit once you pass 90 minutes with the patient (106 minutes for the first unit to be paid) (A185).
- Detention (K001) is not payable for the same patient on the same day (GP31).
- By video only (not phone) with an existing patient relationship; add K300A (Appendix J, A70).
Often billed with
- K630 psychiatric consultation extension (A185).
- K188 (+15%) within 6 months after a suicide attempt (A188).
- K189 for urgent community follow-up within 4 weeks of a psychiatric discharge, when you will keep caring for the patient for at least 6 months (A188).
Common rejections
- ARF: OHIP describes this as 'Missing Physician Referring Number'. A consultation is paid only on a written referral from a physician or nurse practitioner (GP16); enter the referrer's billing number.
- AT4: OHIP describes this as 'Modality Not Allowed': this code is on the video-only list, so it can't be billed with K301A (telephone) (Appendix J).
