Add-on for snare removal of the first polyp over 1 cm during oesophagoscopy-gastroscopy.

What it covers

E703 is added to an oesophagoscopy-gastroscopy for snare polypectomy of the first polyp larger than 1 cm (S7).

Each additional polyp over 1 cm removed by snare is E799, to a maximum of 2 (S7). During a plain gastroscopy (Z527, Z547, Z528) the matching codes are E674 and E675 (S11).

Billing rules

  • Add-on only: bill with oesophagoscopy-gastroscopy (Z399 or Z400) (S7).
  • Only for a polyp larger than 1 cm removed by snare (S7).
  • Endoscopic mucosal resection of a lesion over 3 cm is E836 instead, which needs submucosal injection and piecemeal resection (S7, S8).

Often billed with

  • E799 for each additional polyp over 1 cm, maximum 2 (S7).

Common rejections

  • ADF: The add-on's base procedure (Z399 or Z400) is missing, invalid or paid at zero; bill the add-on on the same claim as a payable base code.
