Explanation

Add-on for each additional polyp over 1 cm removed by snare during oesophagoscopy-gastroscopy, up to 2.

What it covers

E799 is added to an oesophagoscopy-gastroscopy for each additional polyp over 1 cm removed by snare polypectomy, after the first (E703) (S7).

Billing rules

  • Maximum of 2 (S7).
  • Bill with E703 for the first polyp and the oesophagoscopy-gastroscopy (Z399 or Z400) (S7).
  • Only for polyps larger than 1 cm removed by snare (S7).

Often billed with

  • E703 first polyp over 1 cm (S7).

Common rejections

  • ADF: The add-on's base procedure (Z399 or Z400, with E703) is missing, invalid or paid at zero; bill the add-on on the same claim as a payable base code.
  • A3H: More than 2 additional polyps were billed; E799 is limited to a maximum of 2 (S7).
