Add-on to cystoscopy and diagnostic ureteroscopy (Z628) for removing a stone from the ureter or kidney.

What it covers

E760 is added when a calculus (stone) is removed during cystoscopy and diagnostic ureteroscopy above the intramural ureter, Z628 (T4).

Breaking up the stone inside the body is a separate add-on, E761 (T4).

Billing rules

  • Add E761 when intracorporeal lithotripsy is also done (T4).
  • Add E822 when the ureteroscopy reaches the upper third of the ureter or the renal pelvis (T4).
  • Cystoscopy with manipulation or removal of a calculus without ureteroscopy is a different code, S470 (T3).

Often billed with

  • Z628 cystoscopy and diagnostic ureteroscopy (T4).
  • E761 intracorporeal lithotripsy (T4).
  • E822 ureteroscopy to upper third of ureter or renal pelvis (T4).
