E950 is the add-on for inserting an artificial intraocular lens. It is listed twice: with E141 (dislocated lens extraction) and with E144 (removal of an artificial intraocular lens) (Y5).

Billing rules

• Only payable with E141, or with E144 (Y5).
• With E144, only payable when a dislocated implanted lens is removed and replaced during the same surgery (Y5).
• Not payable with E156 or E157 (Y4).
• For routine cataract surgery, E140 already includes lens insertion (Y5).
• Age premiums of 10% to 30% apply for patients under 16 (GP64).

Often billed with

• E141 dislocated lens extraction (Y5).
• E144 removal of artificial intraocular lens, when replaced in the same surgery (Y5).

Common rejections

• ADF: OHIP describes this as 'Corresponding procedure invalid, omitted or paid at zero': this add-on is only paid with E141 or E144 on the same claim.
• ADH: OHIP describes this as 'Cannot be billed together': E950 isn't payable with E156 or E157 (Y4).
