Explanation

Updated October 26, 2026

Add-on to coronary angioplasty (Z434) for each additional major coronary vessel treated.

What it covers

Transluminal coronary angioplasty is Z434 for one or more sites on a single major vessel; G262 is added for each additional major vessel (J11).

Coronary stents are billed separately as G298, per stent (J11).

Billing rules

  • G262 is an add-on: bill it only with Z434 (J11).
  • G262 is exempt from the 50% reduction for additional catheterization procedures at one sitting (J9).
  • It is on the major invasive procedure list for after-hours premiums: add E409 (+50%) or E410 (+75%) when non-elective, or elective but delayed by an emergency into that period; ED physicians billing H codes use E412/E413 instead (GP103–GP104).
  • J058 claimed for the same patient the same day as G298 (stent) is paid at nil (J11).

Often billed with

  • Z434 angioplasty of the first major vessel (J11).
  • G298 coronary stent, per stent (J11).
  • E409/E410 after-hours premium when non-elective (GP104).

Common rejections

  • ADF: The base procedure was missing, invalid or paid at zero; G262 must be billed with Z434 (J11).
