What it covers

G265 is each additional unilateral occipital nerve block, per spinal level per day, following G264 when G264 is payable in full (J80).

Billing rules

  • Maximum 3 per day and 48 additional blocks per calendar year (J80).
  • Paid at nil unless G264 or G291 is payable for the same patient the same day (J80).
  • When G265 is payable, G292 for the same patient the same day is paid at nil (J80).
  • Overall maximum of 8 nerve blocks per patient per day for any combination; the ninth and later are not payable. Bilateral blocks count as two (J79).

Often billed with

  • G264 first occipital nerve block of the day (J80).

Common rejections

  • AD9: OHIP describes this as 'Premium not allowed alone': this add-on must be on the same claim as a payable G264.
  • A3H: More services were billed than OHIP allows; G265 is limited to 3 per day (J80).
