Supervised IV chemotherapy or biologic infusion with toxicities that may need immediate intervention. Includes all assessments for the next 24 hours.
What it covers
G345 is physician-supervised intravenous infusion of complex single-agent or multi-agent chemotherapy and/or biologic agent(s) that can cause vesicant damage, infusion reactions, or cardiac, neurologic, marrow or renal toxicities that may require immediate intervention by the physician (J57). Chemotherapy here means pharmacologic therapy of malignant or autoimmune disease (J57).
The Schedule's examples of complex single agents include rituximab, bevacizumab, trastuzumab, anthracyclines, bortezomib, taxanes, cisplatin, etoposide and fludarabine (J57). Agents with minor toxicity are G381 (standard chemotherapy); agents with major toxicity that need frequent monitoring and prolonged administration are G359 (special therapy) (J57).
Billing rules
- The physician must be available to intervene in a timely fashion at the start and for the whole of the prescribed therapy, to manage immediate and delayed toxicities (J57).
- Includes all patient assessments by any physician for the 24 hours after the treatment is given (J57).
- For biologic agents, payable only for monoclonal antibodies and cytokines. Blood products, insulin and immunizing agents are not biologic agents for this purpose (J57).
- Includes venipuncture, establishing any vascular access line and giving the agent(s), so these aren't billed separately (J57).
