Insulin therapy support: time-based visit for patients on 3+ insulin injections a day or a pump. Maximum 6 units per patient per physician per year.
What it covers
Insulin therapy support (ITS) is an all-inclusive, time-based visit fee per patient per day for assessment, support and counselling of patients on intensive insulin therapy needing at least 3 injections a day or using an infusion pump (A52).
It can include any combination of the common and specific elements of Family Practice & Practice in General services and carries the same minimum time requirements as counselling (A52). It may cover training on glucose meters, insulin pumps and insulin pens when you give it personally (A52).
Billing rules
- Maximum 6 units per patient, per physician, per year (A52).
- Pays nil if you bill any other consultation or visit for the same patient the same day, such as A007 (A52).
- Units are ½ hour or major part: 1 unit = at least 20 minutes, 2 units = 46, 3 units = 76, 4 units = 106 (A52, GP55).
- Record start and stop times in the chart, or the service is not eligible for payment (GP7).
- Training on glucose meters, insulin pumps or insulin pens can be billed as K029 only when you give it personally (A52).
- May be given by video or phone as comprehensive virtual care with K300A or K301A; by phone it pays 85% (AJ1, A70).
- FHO: out of basket (not on Agreement Schedule 2, p.79), and not one of the codes that earn the after-hours premium (Agreement p.68, §8.12).
Often billed with
- K301A or K300A when the visit is by phone or video (A70).
- E080 first visit after hospital discharge, office or home, within 2 weeks of an acute in-patient discharge (GP28).
Common rejections
- EH2: The health card version code doesn't match OHIP's record, often because the patient has a newer card; get the current version code and resubmit (EH2 guide).
