Explanation

Updated October 26, 2026

Annual fee for team-based diabetes care by an internist or endocrinologist working with a Certified Diabetes Educator they employ.

What it covers

Diabetes team management is an annual fee for comprehensive team-based care of a patient with diabetes. The team includes the specialist most responsible for the patient's diabetes care and at least one Certified Diabetes Educator (CDE) (A100).

It covers coordinating, providing and documenting all required elements of ongoing care, by you and/or the CDE (A100).

Billing rules

  • One K046 per patient per 12 months (A100).
  • You must have provided at least 4 consultations or assessments, or K013, K033, K029, K002 or K003, to the patient in the 12 months claimed. Comprehensive virtual care visits count toward these (A100, A75).
  • You must have treated more than 100 patients with diabetes in the period, and be in Internal Medicine (13) or Endocrinology (15) (A100).
  • The CDE must be your employee and currently certified by the Canadian Diabetes Educator Certification Board. Not payable if the CDE is employed by a hospital, public health unit, ICHSC, industrial clinic or long-term care facility (A100, A101).
  • The chart needs a diabetes flow sheet or registry record: lipids, HbA1C, ACR, eGFR or CrCl, blood pressure, height, weight and BMI, medications, prevention of complications, counselling and self-management support, eye exam referral, foot exam and neurological exam. It must name any CDE who did the work (A100, A101).
  • You must document the CDE's services and give them to the Ministry if asked. The CDE doesn't need your physical presence but must report to you (A100, A101).
  • Submit only once the required elements for the previous 12 months are complete (A101).

Often billed with

  • K045 diabetes management by a specialist; K046 is payable in addition to K045 (A101).
  • G500, G514 or G520 diabetes monthly management for insulin-treated patients (J42).

Common rejections

  • A4D: K046 is only payable to Internal Medicine (13) or Endocrinology (15); a claim under another specialty is rejected (A100).
  • AM1: Only one K046 is payable per patient per 12 months; check the date of the last one (A100).
