Explanation

Updated October 26, 2026

Health Status Report completed on its own for an initial ODSP application. When it's done with the ADL Index, bill K050 instead.

What it covers

K051 is the fee for completing the Health Status Report separately for a patient's initial Ontario Disability Support Program (ODSP) application (AF1).

Appendix F is not part of the Schedule of Benefits and is reproduced for information only. Its services are not insured services under the Health Insurance Act; the Ministry of Health pays them as paying agent for the Ministry of Children, Community and Social Services (MCCSS) (AF1).

Billing rules

  • Use K051 only when the Health Status Report is completed separately. When it is completed together with the Activities of Daily Living Index, bill the amalgamated code K050; the ADL Index on its own is K052 (AF1).
  • For the ODSP Medical Review process, the Health Status Report of Part B completed separately is K059, not K051 (AF1).
  • The MCCSS forms are given to patients only by social services staff. Each fee code is specific to its form and can't be claimed for completing any other government document (AF2).
  • FHO Income Stabilization physicians, whose services in the FHO otherwise pay $0 as globally funded, may bill K051A at full fee-for-service value (FHO agreement p.99, Appendix J §3.4–3.6).
