Explanation

Activities of Daily Living Index completed on its own for an initial ODSP application. With the Health Status Report, bill K050 instead.

What it covers

K052 is the fee for completing the Activities of Daily Living Index separately for a patient's initial Ontario Disability Support Program (ODSP) application (AF1).

Appendix F is not part of the Schedule of Benefits and is reproduced for information only. Its services are not insured services under the Health Insurance Act; the Ministry of Health pays them as paying agent for the Ministry of Children, Community and Social Services (MCCSS) (AF1).

Billing rules

  • Use K052 only when the ADL Index is completed separately. When it is completed together with the Health Status Report, bill the amalgamated code K050; the Health Status Report on its own is K051 (AF1).
  • For the ODSP Medical Review process, the ADL Index of Part B completed separately is K060, not K052 (AF1).
  • The MCCSS forms are given to patients only by social services staff. Each fee code is specific to its form and can't be claimed for completing any other government document (AF2).
  • FHO Income Stabilization physicians, whose services in the FHO otherwise pay $0 as globally funded, may bill K052A at full fee-for-service value (FHO agreement p.99, Appendix J §3.4–3.6).
