Fee for Medical Form Part B in the ODSP Medical Review process, covering both the Health Status Report and the ADL Index.
What it covers
K058 is the fee for completing Medical Form Part B for the Ontario Disability Support Program (ODSP) Medical Review process, including both the Health Status Report and the Activities of Daily Living Index (AF1).
Appendix F is not part of the Schedule of Benefits and is reproduced for information only. Its services are not insured services under the Health Insurance Act; the Ministry of Health pays them as paying agent for the Ministry of Children, Community and Social Services (MCCSS) (AF1).
Billing rules
- K058 includes both parts of Part B. When only one is completed, bill K059 (Health Status Report, completed separately) or K060 (ADL Index, completed separately) instead (AF1).
- Medical Form Part A is a separate listing, K057 (AF1).
- The initial ODSP application uses K050 for the amalgamated forms, not K058 (AF1).
- The MCCSS forms are given to patients only by social services staff. Each fee code is specific to its form and can't be claimed for completing any other government document (AF2).
- Unlike K050 to K056, K058 isn't on the FHO agreement's list of codes Income Stabilization physicians may bill at full fee-for-service value (FHO agreement p.99, Appendix J §3.6).
Often billed with
- K057A for Medical Form Part A of the same Medical Review; Part A and Part B are listed as separate services (AF1).
Common rejections
- ADH: OHIP's description is 'cannot be billed together'. The Schedule doesn't name the conflicting code; since K058 already includes both the Health Status Report and the ADL Index of Part B, check the claim for K059 or K060 for the same Part B and bill K058 alone for it (AF1).
