Activities of Daily Living Index of Part B completed on its own in the ODSP Medical Review process. K058 covers it with the Health Status Report.

What it covers

K060 is the fee for completing the Activities of Daily Living Index of Medical Form Part B separately, for the Ontario Disability Support Program (ODSP) Medical Review process (AF1).

Appendix F is not part of the Schedule of Benefits and is reproduced for information only. Its services are not insured services under the Health Insurance Act; the Ministry of Health pays them as paying agent for the Ministry of Children, Community and Social Services (MCCSS) (AF1).

Billing rules

• Use K060 only when the ADL Index of Part B is completed separately. When both the Health Status Report and the ADL Index of Part B are completed, bill K058; the Health Status Report of Part B alone is K059 (AF1).
• Medical Form Part A is a separate listing, K057 (AF1).
• For an initial ODSP application, the ADL Index completed separately is K052, not K060 (AF1).
• The MCCSS forms are given to patients only by social services staff. Each fee code is specific to its form and can't be claimed for completing any other government document (AF2).
• Unlike K050 to K056, K060 isn't on the FHO agreement's list of codes Income Stabilization physicians may bill at full fee-for-service value (FHO agreement p.99, Appendix J §3.6).
