Explanation

Pre-scheduled case conference about a long-term care resident, per 10-minute unit. Maximum 4 per patient per physician in 12 months.

What it covers

A case conference is a pre-scheduled meeting, in person, by video or by phone, to discuss and direct the management of an individual patient (A31). K124 is the most responsible physician's participation, with at least 2 other participants (physicians, regulated social workers, Ontario Health atHome employees or regulated health professionals), about an in-patient of a long-term care institution (A38).

Rounds, journal clubs, group learning and other meetings not about directing one patient's care are not case conferences (A31).

Billing rules

  • Maximum 4 services per patient, per physician, per 12 months, and maximum 8 units per physician, per patient, per day (A38).

  • Payable only if pre-scheduled and you actively participate, with your participation evident in the record (A32).
  • The record must show the patient, the start and stop time of the discussion, the eligible participants and the outcome or decision (A32).
  • Any other insured service during the conference is not payable, and it isn't payable if the required participants are paid by you or if you are paid other than fee-for-service for the conference (A32).
  • Not payable for radiation treatment planning or for supervision of postgraduate trainees (A38).
