Explanation

Updated October 26, 2026

Pre-scheduled case conference about an out-patient's chronic pain (6+ months), led by the physician managing the pain. Per 10-minute unit.

What it covers

A case conference is a pre-scheduled meeting, in person, by video or by phone, to discuss and direct the management of one patient. Teaching rounds and journal clubs are not case conferences (A31).

K707 is the chronic pain out-patient case conference: the physician most responsible for treating the patient's chronic pain plus at least 2 other participants who are physicians, regulated social workers or regulated health professionals (A37). Chronic pain means symptoms lasting at least 6 months (A37).

Billing rules

  • Units are 10 minutes of consecutive time: 1 unit = 10 min, 2 = 16, 3 = 26, 4 = 36, up to 8 units = 76 min (A31).
  • At least 10 minutes of patient-related discussion; you must actively take part and your participation must show in the record (A32).
  • Maximum 4 services per patient per physician per 12 months, and 8 units per patient per day (A37).
  • Not payable with any other case conference or telephone consultation for the same patient that day (A37).
  • For in-patients use K121 (acute, chronic or rehab hospital) or K124 (long-term care) (A37).
  • Not payable if the required participants are paid by you, or if you are paid other than fee-for-service (salary, APP, AFP) for the conference (A32).
  • The record must show the patient, start and stop times of the discussion, the eligible participants and the outcome (A32).

Common rejections

  • AM1: OHIP describes AM1 as 'Service Limit Exceeded': more than 4 K707 services for this patient in 12 months, or more than 8 units in a day (A37).
