Premium for the first patient seen on a non-elective weekday daytime (7:00–17:00) visit to an emergency department. Not for ED physicians.
What it covers
K990 is the Table I first person seen premium for a special visit to an emergency department on weekdays, 07:00 to 17:00 (GP70). Further patients seen on the same visit use K991 (GP70).
On weekday daytime, a first person seen premium is only payable when a travel premium (K960) is also payable, unless you sacrificed office hours (then use K992) (GP66, GP70).
Billing rules
- Maximum 10 first and additional person premiums combined per time period (GP70).
- Not payable to Emergency Department Physicians as defined on GP67 (GP67, GP70).
- When you are not on ED duty and see patients beyond the Table I limits on that visit, bill the extra patients with H-prefix codes (GP68).
- Not payable with H-prefix codes, for walk-ins, for patients seen on rounds or for elective admissions (GP65).
- Bill an A-prefix assessment from the General Listings with the premium, and chart the time of the visit (GP69).
Often billed with
- K960 ED travel premium, weekday daytime (GP66, GP70).
- An A-prefix consultation or assessment from your specialty's General Listings (GP69).
Common rejections
- AD9: 'Premium not allowed alone': submit K990 on the same claim as the A-prefix service it goes with (GP69).
