Explanation

Level 1 oncoplastic breast conserving lumpectomy or partial mastectomy for malignancy: 15% or less of the breast resected, with dual plane undermining.
What it covers
R102 is the level 1 oncoplastic breast conserving lumpectomy or partial mastectomy for malignancy (M26). It requires that the resected breast volume is 15% or less of the ipsilateral breast parenchyma, and that the operative report documents dual plane undermining and closure of the lumpectomy defect (M26).
It includes nipple undermining, nipple-areolar complex elevation or centralization, skin resection and glandular advancement when done (M26). Larger resections, or tumours in the upper inner quadrant or lower pole, are level 2 (R158); resections over 25% using reduction mammoplasty techniques are level 3 (R159) (M27).
Billing rules
- Only payable when the resected volume is 15% or less of the ipsilateral breast parenchyma and the operative report documents dual plane undermining and lumpectomy defect closure (M26).
- Not payable when a second surgeon does another reconstructive procedure on the same breast the same day, or when the procedure is part of a pre-planned staged breast reconstruction (M27).
- R143 contralateral balancing mastopexy or reduction isn't payable on the same day as R102 (M27).
- Add-ons: E525 after wire or radioactive seed localization, E505 limited axillary node sampling, or E546 axillary node dissection up to the axillary vein (M26).
- E505 isn't payable in addition to Z427 sentinel node biopsy; Z427 is payable with E546 only when a frozen section report shows micrometastases (M25).
- Other procedures under the same anaesthetic: the major procedure is paid in full and the others at 85%, unless the Schedule says otherwise (SP2).
- The major pre-operative visit is billed separately; the surgeon can't bill a hospital or day care admission assessment unless it is that visit (GP4, SP2).
- Marked #: premises, equipment, supplies and personnel are not included in the fee (SP1, GP13).
Often billed with
- Z427 sentinel node biopsy per draining basin (R2), subject to the 85% rule for a second procedure under the same anaesthetic (SP2).
- E525 when the lesion was localized with a mammographic wire or radioactive seeds (M26).
- A034 General Surgery partial assessment, which is only billable on the surgery day if it is the major pre-operative visit (GP4, SP2).
