Explanation

Updated October 26, 2026

Excision of breast tumour or tissue for diagnostic biopsy and/or treatment, one or more lesions in the same breast. E525 adds wire or seed localization.

What it covers

R107 is excision of a tumour or tissue of the breast for diagnostic biopsy and/or treatment, for example carcinoma, fibroadenoma or fibrocystic disease (M25).

One fee covers a single lesion or multiple lesions in the same breast (M25). Partial mastectomy or wedge resection for treatment of breast disease is R111, and needle biopsy is Z141 or, for large core biopsy, Z143 (M25).

Billing rules

  • One R107 covers all lesions excised from the same breast (M25).
  • Listed fees are for unilateral procedures; for procedures on both breasts under the same anaesthetic, the major one is paid in full and the other at 85% (SP4, SP2).
  • Add E525 when the excision follows localization with a mammographic wire or radioactive seeds (M25).
  • The major pre-operative visit is billed separately whenever it took place; the surgeon can't bill a hospital or day care admission assessment unless it is that visit (GP4, SP2).
  • For a non-elective procedure done on a special visit, bill the consultation or assessment, the special visit premium and R107; evenings, nights, weekends and holidays also take E409 or E410 (SP2).
  • Marked #: premises, equipment, supplies and personnel are not included in the fee (SP1, GP13).

Often billed with

  • E525 after wire or radioactive seed localization (M25).
  • A034 General Surgery partial assessment, billable on the surgery day only if it is the major pre-operative visit (GP4, SP2).
  • U990 or U991 hospital out-patient special visit premiums, only for a non-elective procedure on a special visit that meets the Table II rules (SP2, GP65, GP71).

Common rejections

  • EH2: The health card version code on the claim doesn't match OHIP's record, often because the patient has a newer card; get the current version code and resubmit.
