Type 3 admission assessment: general re-assessment of a patient re-admitted to LTC after at least 3 days in another institution.
What it covers
A Type 3 admission assessment is a general re-assessment of a patient re-admitted to the long-term care institution after a stay of at least 3 days in another institution (GP49). A general re-assessment has all the parts of a general assessment, but the history can be shorter (GP22).
W-prefix codes apply to patients in chronic care or convalescent hospitals, nursing homes, homes for the aged and designated chronic or convalescent beds, but not designated palliative care beds (GP49).
Billing rules
- Only after the patient has been in another institution for at least 3 days (GP49).
- An admission assessment with a new admission lets W010 qualify that month with fewer than two assessments (GP52).
- Included in W010 when the MRP bills the monthly management fee for that month; don't bill it separately then (GP51, GP52).
- FHO: a Schedule 2A code for enrolled LTC patients (Agreement p.86) and counts toward the long-term care special payment (Agreement p.67 §8.10).
Often billed with
- E079 smoking cessation initial discussion (A56).
