Comprehensive endocrinology consultation in long-term care: at least 75 minutes of direct contact with the patient.

What it covers

A comprehensive endocrinology consultation provides all the elements of a consultation with at least 75 minutes of direct contact with the patient, not counting time on any other separately billable service (A98).

W150 is the long-term care version and has the same conditions as A150 (office); C150 is the hospital in-patient version (A102, A103). W-prefix codes are for non-emergency care in chronic care and convalescent hospitals, nursing homes, homes for the aged and designated chronic or convalescent beds, but not designated palliative care beds (A103, GP49).

Billing rules

  • Record start and stop times in the chart, or payment is reduced to a lesser fee (A98).
  • The 75 minutes excludes time on other separately payable services and non-patient-facing time such as chart, imaging or documentation review (A98).
  • Must meet all the elements of a consultation, including the written request and report (A98, GP16).
  • Counts toward the consultation limits for the same diagnosis (GP17, GP18).
  • Detention (K001) is only payable after 120 minutes (GP31).
  • Patients under 16 get an age premium of 10% to 30%, depending on age (GP64).
  • By video only for an in-patient or long-term care resident, and only if you aren't the MRP, no local physician with the expertise is available (documented by the referrer), and the referrer assessed the patient in person within the past 30 days (Appendix J AJ2, A69).

Often billed with

  • K001 detention after 120 minutes (GP31).

Common rejections

  • ARP: OHIP needs the referring provider's number on a consultation claim; add the referrer's billing number from the written request and resubmit (GP16).
