Additional routine physiatrist visits to a chronic care or convalescent hospital patient after the first 4 in a month. Maximum 6 per month.
What it covers
A subsequent visit is any routine assessment after the patient's admission to a long-term care institution (GP49).
W311 covers additional subsequent visits in a chronic care or convalescent hospital after the first 4 (W312), to a maximum of 6 per patient per month (A179).
Billing rules
- Maximum 6 per patient per month (A179).
- Visits for an acute intercurrent illness that are not special visits are billed as W121, whatever the monthly count; if a special visit is needed, use the General Listings (A prefix) and premiums (GP49).
- If you are already in the institution and see your own patient, use the W codes; seeing another physician's patient on an emergency basis uses the General Listings (A prefix) (GP49).
Common rejections
- AM1: OHIP describes AM1 as 'Service Limit Exceeded': more than 6 W311 visits for this patient in the month (A179).
