Explanation
Updated October 26, 2026
Chemical and/or cryotherapy treatment of one or more skin lesions, once per patient per physician per day. Warts are insured only in some cases.
What it covers
Z117 is chemical and/or cryotherapy treatment of one or more skin lesions (M7). It includes paring or debulking the lesion before or after the treatment (M7).
As a Z code it is an independent operative procedure (IOP), payable in addition to an assessment (GP4). Treatment of warts is insured only in the cases set out in Appendix D (M7, AD4).
Billing rules
- Maximum one Z117 per patient per physician per day, however many lesions are treated (M7).
- Warts are insured only when they are plantar (by any listed procedure except excision or electrocoagulation and/or curetting), perianal, genital, any wart in an immunocompromised patient, or on the head or neck of an infant or child (AD4).
- Benign lesions removed for cosmetic reasons, without clinical suspicion of disease, are not insured and are billed to the patient (AD3).
- If you did the major pre-operative visit in the previous 12 months, only a minor assessment (GP/FP) or partial assessment (specialist) is payable before the IOP that day (SP4).
- If the decision to treat is made at that day's visit, that visit is the major pre-operative visit and is billable; no other consultation or assessment before the IOP that day (GP4, SP4).
- An unrelated, significant problem in another body system assessed at the same visit can be billed as well (SP4).
- Age premiums apply to surgical procedures for patients under 16 (GP64).
- FHO: a Schedule 2B core service (50% shadow billing for enrolled patients) and a Schedule 4 office procedure (Agreement p.59 §4.4, p.87, p.67 §8.7, p.94).
Often billed with
- A minor or intermediate assessment (A001/A007) at the visit where the decision to treat is made (GP4, SP4).
- Z116 biopsy, which may be allowed with Z117 (M1).
Common rejections
- A3H: Z117 is limited to one service per patient per physician per day (M7), so billing more than one service, for example one per lesion, rejects.
- EH2: The health card version code on the claim doesn't match OHIP's record, often because the patient has a newer card; get the current version code and resubmit.
- VJ7: OHIP received the claim more than three months after the service date, so it was rejected as stale-dated (GP9).
