- Description
- Fee code billed is not supported by the documentation provided
Explanation
Updated October 26, 2026
Code 41 means the documents sent with the claim didn't support the fee code billed, so it wasn't paid as billed. Check them against the code's requirements.
What it means
Code 41 is a remittance advice explanatory code. The Ministry's wording is 'Fee Schedule Code (FSC) Billed - No Evidence in Supporting Documentation Provided'. OHIP reviewed the records sent for the claim and found nothing showing the service the fee code describes.
It follows claims that needed supporting documentation, such as claims flagged for manual review, independent consideration (IC) claims, or claims where the Ministry asked for an operative report.
Many codes have medical record requirements. For example, a service with a minimum time is not eligible for payment unless the chart records when it started and ended (GP7).
Common causes
- The operative report or notes sent don't describe the procedure billed
- Start and stop times missing for a time-based service
- The wrong document sent, or one for another patient or date
- A higher-paying code billed than the records support
How to fix it
- Compare the documents you sent with the code's description and medical record requirements in the Schedule of Benefits.
- If the records support a different code, ask for the claim to be reassessed under that code through an RAI.
- If the right documents weren't sent, or you disagree with the decision, send a Remittance Advice Inquiry (RAI) through eSubmit, or on form 0918-84, within 7 months of the RA date, with the complete records and a short explanation.
- For future claims that need documentation, flag them for manual review and send the documents through eSubmit on the same day you submit the claim.
Don't resubmit if
- The records don't show the service billed.
- You'd be sending the same claim again without new documentation. It will get the same result.
- The claim is on the RA. Send the documents with an RAI, not a new claim.
How APEX handles it
APEX has no correction rule for code 41 and doesn't send documentation or inquiries for it. If nothing on the visit was paid, the claim is listed under Outstanding for you to review. APEX can't see what documentation the Ministry received. See how APEX works · Pricing
