- Code
- 48
- Description
- Paid as submitted - clinical records may be requested
Explanation
Updated October 26, 2026
OHIP paid the claim as submitted, but may ask for your clinical records to check it. Keep the records on file.
What it means
Code 48 means the claim was paid as submitted. The Ministry notes that clinical records may be requested later for verification.
Nothing has been reduced. The code is a reminder that the service may be audited.
How to fix it
- Make sure the patient's chart documents everything the fee code requires, including start and stop times for time-based codes (General Preamble GP7).
- Respond to any records request from the Ministry promptly.
Don't resubmit if
- The claim was paid as submitted. Sending it again would be a duplicate.
