What to do: Find out who else billed it; if their claim is correct, don't resubmit.

Code
A14
Description
Service already provided by another practitioner, group or IHF

Explanation

Updated October 26, 2026

OHIP shows this service was already provided by another practitioner, group or IHF, so the claim was rejected.

What it means

A14 means OHIP's records show the service was already rendered by another practitioner, group or independent health facility (IHF). The claim was rejected rather than paid.

The same situation on the Remittance Advice is explanatory code I4.

Common causes

  • The patient had the same service from another physician or facility
  • The service was billed under both a group number and a solo number
  • A diagnostic service was billed by the facility that did it

How to fix it

  1. Check the patient, service date and fee code.
  2. Find out who else billed the service.
  3. If the other claim is correct, don't resubmit.
  4. If you provided the service and the other claim is wrong, call the Service Support Contact Centre (1-800-262-6524) before resubmitting.

Don't resubmit if

  • Another provider or facility rightly billed the service.
