Error code

I4 — Service already provided by another practitioner, group or IHF

Paid to someone else

OHIP already paid this service for this patient to another physician, group or facility, so your line was reduced or not paid.

What to do: Find out who billed the same service; if they billed it in error, they need to have it reversed.

Code
I4
Description
Service already provided by another practitioner, group or IHF

Explanation

Updated October 26, 2026

I4 means OHIP has already paid this service to another physician, group or facility. Check who billed it before asking for a review.

What it means

I4 appears on your Remittance Advice (RA) when the Ministry's records show the same service for this patient was already paid to another practitioner, a group, or an Independent Health Facility (IHF). Your line is reduced or not paid.

Independent Health Facilities are now called Integrated Community Health Services Centres (ICHSCs). The Ministry still uses the older name in this message.

Common causes

  • Another physician, or your own group, already billed the same service for the patient on that date
  • A facility billed a component of the service (for example the technical part) that your claim also covered
  • The claim has the wrong patient, date or fee code

How to fix it

  1. Check the line on the RA: the patient, service date and fee code.
  2. Find out whether a colleague, your group or a facility billed the same service. If someone else billed it in error, they need to have it reversed.
  3. If your claim had the wrong date, patient or code, note the correct details for the inquiry.
  4. If you believe the decision is wrong, send a Remittance Advice Inquiry (RAI) through eSubmit, or on form 0918-84, within 7 months of the RA date, with supporting details.

Don't resubmit if

  • The line is already on your RA. Question it with an RAI, not a new claim.
  • Another provider correctly billed and was paid for the same service.
