Date of birth doesn't match

What to do: Check the date of birth on the patient's card or a validation check, correct it in the chart and claim, and resubmit.

Code
VH8
Description
Date of birth does not match health number

Explanation

Updated October 26, 2026

VH8 means the patient's date of birth on the claim doesn't match OHIP's record for that health number. Correct the date of birth and resubmit.

What it means

OHIP checks the date of birth on each claim against the date it has on file for the health number. If they differ, the claim is rejected with VH8. The Ministry's full wording is that the date of birth does not match the health number submitted.

This is usually a data entry problem, not a coverage problem. Once the date of birth matches, the claim can go through normal processing.

Common causes

  • Date of birth typed wrong, for example day and month swapped or the wrong year
  • Wrong health number on the claim, so the date of birth belongs to a different person
  • A family member's card or chart used by mistake
  • Date of birth never updated after the patient corrected it with ServiceOntario

How to fix it

  1. Check the date of birth on the patient's card or ID, or run a health card validation check.
  2. If the date of birth on the claim is wrong, correct it in the chart and on the claim.
  3. If the date of birth is right, check the health number. The number may belong to someone else.
  4. Resubmit the corrected claim. Resubmit within three months of the service date if you can. If that window has passed but the claim was first submitted on time, use the Ministry's stale-dated resubmission process: one stale-dated claim file through MCEDT, then an email to [email protected] with the Error Reports showing the original submission date.

Don't resubmit if

  • You haven't changed the date of birth or health number. The same claim will reject again.
  • The patient says OHIP's record of their date of birth is wrong. They need to fix it with ServiceOntario first.

How APEX handles it

APEX treats VH8 as an automatic fix. It takes the patient's date of birth from its own records or a health card check and puts it on a corrected claim. You can review the claim before it's sent, or it goes out with auto-fix if you've turned that on. See how APEX works · Pricing
