Code
A3G
Description
Fee Billed Low

Explanation

Updated October 26, 2026

The fee billed is lower than the Schedule fee for this code. Bill the current Schedule amount and resubmit.

What it means

A3G means the fee billed is low. The amount on the claim is less than OHIP expects for the code and number of services.

The Ministry's matching Error Report message 14 says: fee billed low, check for current Schedule of Benefits fee.

Common causes

  • An old fee left in the billing software after a Schedule change
  • The fee not multiplied by the number of services or units
  • A premium or percentage add-on worked out wrongly

How to fix it

  1. Look up the current fee for the code on the service date.
  2. Multiply by the number of services or units and correct the fee billed.
  3. Update the fee in your billing software so later claims are right.
  4. Resubmit the corrected claim. Resubmit within three months of the service date if you can. If that window has passed but the claim was first submitted on time, use the Ministry's stale-dated resubmission process described under VJ7.

Don't resubmit if

  • The fee hasn't been changed. It will reject again.
