Error code
V41 — Invalid Fee Billed
Invalid fee submitted
The fee submitted is missing or invalid; since May 2026 it also flags FHO hourly codes Q310A to Q313A billed at $0.00.
What to do: Enter the Schedule fee multiplied by the number of services, so it divides evenly, and resubmit.
- Code
- V41
- Description
- Invalid Fee Billed
Explanation
Updated October 26, 2026
V41 means the fee submitted on the claim is missing or invalid. Since May 2026 it is also used for FHO hourly codes billed at $0.00.
What it means
Every claim line carries a fee submitted. OHIP's claim format needs six digits in dollars and cents, from 000000 to 500000 ($0.00 to $5,000.00). OHIP rejects the claim with V41 if the fee is missing, isn't six digits, or is outside that range.
Since May 1, 2026, the Ministry also rejects FHO hourly-rate codes (Q310A to Q313A) billed at $0.00 with V41, for service dates on or after April 1, 2026. INFOBulletin 260501 says these codes must be billed at the dollar value that matches the number of units claimed.
Common causes
- The fee field was left blank.
- One line was billed at more than $5,000.00.
- The billing software sent the fee in the wrong format.
- An FHO hourly code (Q310A to Q313A) was billed at $0.00.
How to fix it
- Check the fee submitted on the rejected line.
- Enter the Schedule of Benefits fee for the code multiplied by the number of services. The total must divide evenly by the number of services, or the claim rejects with V47.
- For Q310A to Q313A, bill the dollar amount for the units claimed, using the unit and payment rules in INFOBulletin 260309.
- If a single line would be more than $5,000.00, call the Service Support Contact Centre (1-800-262-6524) before resubmitting.
- Resubmit the corrected claim. If more than three months have passed since the service date and the original claim was sent on time, use the stale-dated claim process described under VJ7.
Don't resubmit if
- The fee hasn't changed. It will reject again.
- It is an hourly code still billed at $0.00 for a service date on or after April 1, 2026.
How APEX handles it
APEX has no automatic fix for V41. It lists the claim in the Correction Centre as needing your input, so you can correct it before it is resubmitted. See how APEX works · Pricing
