Code
V42
Description
Invalid Number of Services

Explanation

Updated October 26, 2026

V42 means the number of services on the claim is missing or invalid. It must be two digits from 01 to 99.

What it means

Each claim line says how many times the service was provided, or how many units for codes billed in units. OHIP's claim format needs two digits between 01 and 99. OHIP rejects the claim with V42 when the number of services is missing, isn't two digits, or is outside that range (for example 00).

The number of services must also divide evenly into the fee submitted. If it doesn't, the claim rejects with V47 instead.

Common causes

  • The number of services was left blank or entered as zero.
  • A value of 100 or more was entered.
  • The billing software sent the value in the wrong format.

How to fix it

  1. Check how many services or units were actually provided.
  2. Enter that number as two digits between 01 and 99.
  3. Recheck the fee submitted so it equals the per-service fee times the number of services.
  4. If you need to bill more than 99 services or units, call the Service Support Contact Centre (1-800-262-6524) for how to submit it.
  5. Resubmit the corrected claim. If more than three months have passed since the service date and the original claim was sent on time, use the stale-dated claim process described under VJ7.

Don't resubmit if

  • The number of services hasn't changed. It will reject again.
  • The services were already paid on another claim.

How APEX handles it

APEX has no automatic fix for V42. It lists the claim in the Correction Centre as needing your input, so you can correct it before it is resubmitted. See how APEX works · Pricing
