Error code
A3I — Maximum number of X-ray services reached
Too many X-ray services
The X-ray line has more services than OHIP accepts for that code, so the claim was rejected and nothing was paid.
What to do: Check what one service covers in the code's listing, correct the number of services and fee, and resubmit.
- Code
- A3I
- Description
- Maximum number of X-ray services reached
Explanation
Updated October 26, 2026
A3I means the claim billed more services of an X-ray code than OHIP accepts for that code. Correct the number of services and resubmit.
What it means
The Ministry's Fee Schedule Master (FSM) sets the maximum number of services it accepts for each fee code. A3I is the X-ray version of A3H: the number of services on a diagnostic radiology line is more than the FSM allows for that code. The claim was rejected to your Error Report and nothing was paid.
The Schedule of Benefits sets how X-ray services are counted. Fees are for one side unless the listing says otherwise, and a stereo pair counts as two views (D2). Some listings carry their own cap: X180, selective angiography using single films, is billed per vessel to a maximum of 4 (D15). Extra views are billed with the listed 'each additional film or view' code where there is one, such as X081 (D9).
The Ministry doesn't publish the FSM limit for each code, so the Schedule listing is the best guide to what was allowed.
Common causes
- The number of services set higher than the code allows, for example views counted as services on a code that already covers them
- A per-vessel or per-view code billed above its listed maximum
- The number of services keyed wrong
How to fix it
- Read the Schedule listing for the code: what one service covers, any maximum, and whether there is an 'each additional' code.
- Reduce the number of services to what was provided and allowed, and adjust the fee to match.
- If you believe more services were provided and payable, call the Service Support Contact Centre (1-800-262-6524) before resubmitting. Don't split the services across extra claims to get around the limit.
- Resubmit the corrected claim within three months of the service date if you can. If that window has passed but the claim was first submitted on time, use the Ministry's stale-dated resubmission process: one stale-dated claim file through MCEDT, then an email to [email protected] with the Error Reports showing the original submission date.
Don't resubmit if
- The number of services hasn't changed. It will reject again.
How APEX handles it
APEX has no automatic fix for A3I. It lists these claims in the Correction Centre under Outstanding for you to correct. Its A3H rule, which resets the number of services to 1, doesn't apply to A3I. See how APEX works · Pricing
