Error code
AD5 — Procedure already allowed
Procedure already allowed
A matching procedure is already on file for this patient, so this one was rejected.
What to do: If the earlier procedure was paid, don't resubmit; for a separate repeat, check for a repeat code.
- Code
- AD5
- Description
- Procedure already allowed
Explanation
Updated October 26, 2026
OHIP has already allowed this procedure for the patient, so the claim was rejected.
What it means
AD5 means the procedure was allowed previously. A matching procedure is already on file for the patient, so this one was rejected.
Common causes
- The procedure was already billed, by you or another physician
- A duplicate claim
- A repeat procedure billed with the code for the first procedure
How to fix it
- Check the patient's history for the same procedure.
- If it was already paid, don't resubmit.
- If it was a separate repeat procedure, check whether the Schedule has a repeat code or allows it, and bill that.
- Resubmit the corrected claim. Resubmit within three months of the service date if you can. If that window has passed but the claim was first submitted on time, use the Ministry's stale-dated resubmission process described under VJ7.
Don't resubmit if
- The procedure was already paid.
