Code
A34
Description
Multiple duplicate claims

Explanation

Updated October 26, 2026

OHIP received this claim more than once. Find the original before sending anything again.

What it means

A34 means OHIP received multiple duplicate claims for the same service. The extra copies were rejected.

Common causes

  • The same claim submitted twice, for example from two billing systems
  • A claim resent before the first one appeared on a Remittance Advice
  • A corrected claim sent while the original was still being processed

How to fix it

  1. Search your billing history for the same patient, service date and fee code.
  2. If the original was paid, or is still pending, do nothing.
  3. If you really provided the same service more than once that day, check the Schedule allows it, then resubmit with the manual review indicator and documentation of both services.

Don't resubmit if

  • The original claim was paid or is still being processed.
  • You haven't found the original yet.

How APEX handles it

APEX reads your error reports and remittance advice from MCEDT and lists claims rejected with this code in its Correction Centre for you to correct and resubmit. See how APEX works · Pricing
