AT3 โ€” No Patient-Physician Relationship

No existing patient relationship

OHIP found no existing patient-physician relationship on the service date, which comprehensive virtual care (K300A or K301A) requires.

What to do: Check for an in-person insured visit with you in the past 24 months, or enrolment to you or your group.

Code
AT3
Description
No Patient-Physician Relationship

Explanation

Updated October 26, 2026

AT3 means OHIP rejected a comprehensive virtual care claim because it found no existing patient-physician relationship on the service date.

What it means

Since December 1, 2022, comprehensive virtual care (video or phone visits billed with the K300A or K301A modality codes) is only payable when you already have an ongoing relationship with the patient. When OHIP's records don't show one, the claim rejects to your Error Report with AT3.

Under the Ministry's billing brief, the relationship exists if, in the previous 24 months, you saw the patient in person for an insured service, or if the patient signed an enrolment form and is enrolled to you or to another physician in the same primary care enrolment group. Specialists and focused-practice physicians have some extra ways to qualify, such as a prior video consultation.

Visits to patients with no relationship can only be billed as limited virtual care (A101A video, A102A phone). Limited virtual care does not create a relationship.

Common causes

  • The patient has never been seen in person by you, or not in the last 24 months.
  • The patient is enrolled with you, but the enrolment (Q200A) had not been processed when the virtual claim was assessed.
  • The patient was enrolled to another group, or their enrolment ended before the service date.
  • The in-person visit that creates the relationship was billed under a different physician.

How to fix it

  1. Check whether the patient had an in-person insured visit with you in the 24 months before the service date.
  2. Check your latest roster report to see whether the patient was enrolled to you or your group on that date.
  3. If the patient signed an enrolment form but isn't on the roster yet, make sure the enrolment claim has been submitted and accepted.
  4. Once the claim that establishes the relationship is approved, resubmit the virtual claim unchanged.
  5. If no relationship existed, resubmit the visit as limited virtual care (A101A or A102A) if it meets those conditions.

Don't resubmit if

  • The relationship still isn't on OHIP's records. The same claim will reject AT3 again.
  • The patient has no in-person visit with you in the past 24 months and isn't enrolled to your group. Bill limited virtual care instead, if appropriate.
  • The claim is past the three-month submission window, unless you send it as a stale-dated claim.

How APEX handles it

For family practice claims, APEX resubmits AT3 claims as billed, with the patient's date of birth taken from a health card check. Where the physician has a group number to bill under, APEX also sends a Q200A rostering claim for the same visit. Outstanding rows are tagged when the roster report doesn't show the patient or when APEX finds no earlier in-person visit. See how APEX works ยท Pricing
