- Code
- EF4
- Description
- Provider not approved to bill ICHSC fees on service date
Explanation
Updated October 26, 2026
EF4 means the provider wasn't approved to bill ICHSC fees on the service date. Check the provider's approval and the date.
What it means
EF4 means the health care provider on the claim was not approved to bill an Integrated Community Health Services Centre (ICHSC) fee on the service date, so the claim was rejected.
Common causes
- The provider's approval with the ICHSC hadn't started yet, or had ended, on the service date
- The wrong provider number on the claim
- The wrong service date on the claim
How to fix it
- Check the provider number and service date on the claim.
- Confirm with the facility that the provider was approved to bill its fees on that date.
- Correct the claim and resubmit. Resubmit within three months of the service date if you can. If that window has passed but the claim was first submitted on time, use the Ministry's stale-dated resubmission process.
Don't resubmit if
- The provider wasn't approved on the service date.
