Error code

EH4 — Service Date after Eligibility End Date

Coverage had ended

The service date is after the patient's OHIP coverage ended, so OHIP treats them as not covered that day.

What to do: Check the health number and service date against the chart, then run a health card validation check.

Code
EH4
Description
Service Date after Eligibility End Date

Explanation

Updated October 26, 2026

EH4 means the patient's OHIP coverage had ended before the service date. Confirm the health number, then the patient's coverage.

What it means

OHIP keeps an end date when a person's coverage stops. EH4 means the claim's service date is after that end date, so OHIP treats the patient as not covered on the day of the visit.

Coverage depends on the patient's status, primary residence in Ontario and time spent in the province. When those change, coverage can end. EH4 can also be a data problem, such as a wrong health number or service date on the claim.

Common causes

  • Wrong health number on the claim, such as a family member's or an old number
  • Service date entered wrong
  • Patient moved out of Ontario or no longer meets OHIP's residency or status rules
  • Patient's registration lapsed and hasn't been updated with ServiceOntario

How to fix it

  1. Check the health number and service date on the claim against the chart and the patient's card.
  2. Run a health card validation check to see whether the patient is currently eligible.
  3. If the number or date was wrong, correct it and resubmit. Resubmit within three months of the service date if you can. If that window has passed but the claim was first submitted on time, use the Ministry's stale-dated resubmission process: one stale-dated claim file through MCEDT, then an email to [email protected] with the Error Reports showing the original submission date.
  4. If the details are right, ask the patient to contact ServiceOntario about their coverage. The Error Report may also show message 12, which asks you to advise the patient to contact the Ministry about eligibility, card status or address.
  5. If the patient's coverage is restored for the service date, resubmit the claim. If not, OHIP won't pay it.

Don't resubmit if

  • Nothing on the claim has changed and the patient's coverage hasn't been restored for the service date.
  • The patient was not covered on that date. OHIP will keep rejecting the claim.

How APEX handles it

APEX treats EH4 as an eligibility problem, not an automatic fix. It parks these claims on the same eligibility (EH2) table it uses for version-code problems, so the patient's coverage can be checked before anything is resent. See how APEX works · Pricing
