Code
EH9
Description
Health Number (HN) Not Activated

Explanation

Updated October 26, 2026

EH9 means the patient's health number isn't activated yet. The patient needs to contact ServiceOntario; resubmit once it's active.

What it means

EH9 means the health number on the claim exists but has not been activated, so the claim was rejected.

The patient may be newly registered, or a registration step may be incomplete. For newborns, the Ministry's Error Report message 25 asks the parent or guardian to contact the Ministry when a newborn registration is incomplete.

Common causes

  • A new registration that hasn't been completed
  • An incomplete newborn registration
  • A typing error that produced another, inactive health number

How to fix it

  1. Check the health number against the patient's card.
  2. Run a health card validation check for the service date, or ask the patient for their current card.
  3. If the number was mistyped, correct it and resubmit.
  4. Otherwise, ask the patient (or a newborn's parent or guardian) to contact ServiceOntario to complete registration.
  5. Resubmit once the health number is active. Resubmit within three months of the service date if you can. If that window has passed but the claim was first submitted on time, use the Ministry's stale-dated resubmission process.

Don't resubmit if

  • The health number still isn't active.
