Error code

VH9 — Health number not registered with the Ministry

Health number not registered

The health number has the right format but no registered patient matches it, often from a copying error or incomplete registration.

What to do: Compare the number with the patient's current card digit by digit and run a health card validation check before resubmitting.

Code
VH9
Description
Health number not registered with the Ministry

Explanation

Updated October 26, 2026

VH9 means the health number on the claim isn't registered with the Ministry. Check it against the patient's card and resubmit.

What it means

VH9 means OHIP can't find the health number on the claim in its registration records. The number has the right format, but no registered patient matches it.

This is often a transcription error that still produces a believable number. It can also happen when a patient's registration was never completed, for example a baby whose infant registration form wasn't submitted.

Common causes

  • Health number copied wrong from the card or an old record
  • Number from a referral letter or form rather than the patient's own card
  • Patient's OHIP registration not completed
  • Newborn's pre-assigned health number not yet registered

How to fix it

  1. Compare the health number with the patient's current card, digit by digit.
  2. Run a health card validation check to confirm the number is registered.
  3. If you can't get the card, physicians registered as health information custodians can request the number through ServiceOntario's Health Number Release service.
  4. If the patient isn't registered, ask them (or the parent, for a newborn) to contact ServiceOntario. For a newborn, the infant registration form must be submitted for the number to be activated.
  5. Resubmit once you have a registered number. Resubmit within three months of the service date if you can. If that window has passed but the claim was first submitted on time, use the Ministry's stale-dated resubmission process: one stale-dated claim file through MCEDT, then an email to [email protected] with the Error Reports showing the original submission date.

Don't resubmit if

  • You haven't changed the health number, or the patient still isn't registered.
  • The Ministry's guidance says no claim is payable if updated health card information can't be obtained.
