Code
EP6
Description
Health Number (HN) Not Activated

Explanation

Updated October 26, 2026

EP6 means the patient's health number isn't activated, so the enrolment claim can't be processed. Resubmit once it's active.

What it means

EP6 is the patient enrolment version of EH9. It means the health number on the claim has not been activated, so the claim was rejected.

Common causes

  • A new or incomplete registration, including newborns
  • A typing error in the health number

How to fix it

  1. Check the health number against the patient's card.
  2. Run a health card validation check for the service date, or ask the patient for their current card.
  3. If the number was mistyped, correct it and resubmit.
  4. Otherwise, ask the patient (or a newborn's parent or guardian) to contact ServiceOntario, and resubmit once the number is active.

Don't resubmit if

  • The health number still isn't active.
