Explanation
Updated October 26, 2026
EPF means the claim's date doesn't match the patient's enrolment date. Check the date on the enrolment form and resubmit.
What it means
EPF means there is an enrolment date mismatch: the date on the claim doesn't match the patient's enrolment date in the Ministry's records. The claim was rejected.
For an enrolment code such as Q200A, the service date must be the date on the Patient Enrolment and Consent to Release PHI form (OMA Q-codes guide p.9 to 10). The Remittance Advice message I7 reads 'Claim date does not match patient enrolment date'.
Common causes
- The claim used the visit date instead of the date on the enrolment form
- A typing error in the service date
How to fix it
- Find the patient's signed enrolment and consent form.
- Correct the service date so it matches the form.
- Resubmit.
Don't resubmit if
- The date already matches the form. Check the patient's enrolment status instead.
How APEX handles it
APEX reads your error reports and remittance advice from MCEDT and lists claims rejected with this code in its Correction Centre for you to correct and resubmit. See how APEX works · Pricing
